{t:"2008. On September 16, 2008 Mr. McBride requested a second rescheduling until",p:22,x:167,y:1190,w:606,h:20},
{t:"November 6, 2008, which was again granted. On November 5, 2008 Mr. McBride",p:22,x:167,y:1212,w:608,h:20},
{t:"called the examiner and requested still another delay and a new examination date of",p:22,x:167,y:1234,w:621,h:20},
{t:"December 15, 2008 was agreed to. On December 15, 2008 the examiner appeared at",p:22,x:167,y:1256,w:625,h:20},
{t:"the business address of record with the Department to find a vacated office suite. The",p:22,x:167,y:1278,w:632,h:20},
{t:"property manager confirmed that McBride had moved out of the office and left no for-",p:22,x:167,y:1300,w:641,h:20},

{t:"4.    In accordance with Department policy the Department issued a 5 rated examina-",p:22,x:167,y:1344,w:630,h:20},
